Patient Ally Portal

How the Patient Portal Works

  1. Once you have made an appointment with our office, we will send you a link for our Patient Ally Link by email that you provided us.
  2. When you receive the link, you simply click on the link and enroll in the portal.  
  3. Once this is done and you are enrolled, a message is sent to us that you are able to get online with our office to view your records, labs, send and receive messages and view your last visits.  You can also pay your bill via the portal.
  4. Once we get the message that you are enrolled, we will send you all the intake forms that you are required to fill out prior to seeing the nurse practitioner.  
  5. You click on “documents” under your profile and fill the documents out, and hit send.  It’s that easy. 
  6. If these documents are not filled out here, then you will have to fill them out when you get to the office. 

Please Bring the following to the office:

  1. A list of all your medication that you are currently taking with the doses and the directions.
  2. If you are unable to make a list, bring your bottles, so that we can read the scripts
  3. Bring your drivers license or id, and you insurance cards.  We have to verify this at each visit. 
  4. If it is your first visit, be sure on your history that you list all past illness as well as the current.
